Liberty Advanced Nursing provides the advanced nursing at home that Maryland patients with complex medical, post-hospital, chronic, and extended-care needs require. An RN reviews every case, the plan of care is coordinated with the patient's physician, and qualified nurses are matched to the patient's clinical and scheduling needs. Services are provided when authorized, clinically appropriate, within Liberty's licensure and payer approval, and when qualified staffing is available.

A Liberty RN reviews the patient's diagnosis, orders, equipment, location, timing, and payer before care is arranged, so families and referral sources get a clear, realistic response about what can be provided.

Skilled nursing per the plan of care

Care follows a written, physician-ordered plan. Nurses document assessments, deliver the ordered interventions, and communicate notable changes to the physician and the family.

Clinical coordination

Liberty coordinates with physicians, discharge planners, suppliers, and therapists so the medical, equipment, and scheduling pieces of a transition fit together rather than competing.
