From First Call to Start of Care, in Six Clear Steps
Families arrive with different questions and different levels of certainty. This is the path Liberty walks with them, so the process feels coordinated rather than overwhelming.

1 · Call or Submit a Request
Families call 240-376-2550 or request care through the Start Care page. A member of the care team listens to the situation and confirms the basics: who, where, what needs, and timing.

2 · Clinical Review
A registered nurse reviews the patient’s needs, physician orders or plan of care, equipment, location, payer, and the care structure being considered.

3 · Coverage Verification
Liberty verifies authorization and coverage with the payer, or confirms a private-pay arrangement, before any service is committed. All coverage is confirmed on a case-by-case basis before care begins.

4 · Assessment
When appropriate, Liberty coordinates an in-home assessment so the plan is built around the actual home environment, the family, and the patient’s condition.

5 · Individualized Plan of Care
The plan of care is written, physician-ordered where nursing is involved, and shared with the family. Everyone knows who does what, when, and how to escalate.

6 · Start of Care
Qualified nurses are matched to the patient’s clinical and scheduling needs, and care begins. Liberty stays accessible as the plan evolves.
