Liberty's RNs review the discharge summary, orders, equipment needs, and payer coverage as soon as a referral is received, then respond about whether and how care can be arranged.

Before start of care, Liberty works with the family to confirm the home can safely support the patient: equipment placement, a clear bed space, medication organization, and the modifications that make a transition work.

Readmission prevention

Nurses watch for the early warning signs that often precede a return to the hospital: medication errors, wound changes, respiratory distress, and failure to follow the plan of care.
