Neurological and Chronic-Condition Care

Neurological and chronic conditions change day to day, and the family may not see the warning signs until a crisis. Liberty Advanced Nursing provides neurological and chronic-condition care in Maryland: seizure monitoring, medication administration, and skilled observation for patients with conditions such as seizure disorders, neuromuscular disease, and advanced heart, lung, or kidney disease. Care is delivered per the physician's plan of care and reviewed by an RN for clinical appropriateness and available qualified staffing.

NEUROLOGICAL AND CHRONIC-CONDITION CARE
01

Neurological monitoring

Nurses observe for seizure activity, changes in level of consciousness, and medication side effects, and they follow the patient's seizure action plan without delay.

02

Chronic-condition watch

For patients with heart failure, COPD, diabetes, or kidney disease, nurses track the signs that predict decompensation and report them early.

03

Medication support

Complex regimens are administered on schedule, monitored for effect, and kept reconciled so no prescriber is working from an outdated list.

Who This Fits

Neurological and chronic-condition care fits children and adults with seizure disorders, progressive neurological disease, and chronic conditions that have landed a family member in the hospital more than once.

Services are provided when authorized, clinically appropriate, within Liberty's licensure and payer approval, and when qualified staffing is available. This page is educational and is not medical advice.
