Before the Patient Comes Home
Print this before the discharge conference. Each item is here to prevent a specific problem later.

Confirm the discharge date with the case manager before choosing a start date

Ask for the reconciled medication list in writing
- 3
Confirm equipment delivery dates with each supplier
- 4
Prepare the patient’s room: bed placement, power, supplies, and a call bell
- 5
Clear paths from bed to bathroom and entry of rugs and cords
- 6
Post emergency numbers where the household can see them
- 7
Plan who covers the first night and who the family calls at 2 a.m.

Have the physician’s order and plan of care ready for the agency review

Ask about payer authorization before start of care, not after

Tell the Care Team About the Situation
Share what you know today: the patient's location, the level of nursing you think may be needed, and the county where care would be provided. A nurse will respond and take it from there.
Questions to have ready: discharge date, diagnosis, requested schedule, equipment, payer, and whether a physician's order exists. If you do not know, just say so.
Service areas: Montgomery County, Prince George's County, and Southern Maryland. Confirm availability for a specific location.
