Referrals receive RN-led clinical review, a clear response, and coordinated start-of-care planning. This page explains what to send, what will be reviewed, and what happens after submission.

How the Secure Process Works

A Liberty team member confirms the secure method for sending patient information and documents, so protected health information never travels through ordinary channels.

A nurse reviews the needs, location, timing, payer, orders, and staffing requirements, then responds with a clear answer.

Confirmation and Tracking

You receive a confirmation of receipt, what happens next, and a direct contact for urgent questions. Submission does not guarantee acceptance.

Privacy and security. Liberty treats protected health information with the care the law requires. Vendors that handle PHI on Liberty's behalf are required to sign Business Associate Agreements, and patient documents are never routed through ordinary email notifications.

History and physical

The clinical baseline for the plan of care.

The reconciled medications and administration details.

What Happens After You Submit

After the referral is received, the referral source is given:

  • A confirmation number and the time and date the referral was received
  • A clear explanation of what happens next, step by step
  • A direct contact for urgent questions during the review
  • A clear statement that submission does not guarantee acceptance or start of care
